Send Flowers to the Family Offer Condolences or MemoryPATRICK (JACK) JOHN KOTAK Sandy Hook, Manitoba It is with great sadness we announce the passing of Jack on Wednesday, July 29, 2009 at the age of 89 years. He was born on July 6, 1920 in Grandview, Manitoba. He served in the Canadian Army in the First Division with the First Infantry Brigade and the 8th Army in the Mediterranean and Italy Campaign. Upon his discharge from the army, he was employed with the T. Eaton Co. for many years. Among his many achievements, Jack was Past President and Life Member of the Fort Rouge Branch of the Royal Canadian Legion #97. He also received the distinction of Honourary President of the Branch. He was a recipient of the 55 year pin and the Certificate of Merit Award for this outstanding contribution towards the betterment of the Branch. He also received a 60 year pin in recognition of his service time and work. Furthermore, he served as a member of the Lord Selkirk Masonic Lodge #137 and Khartum Temple of the Shrine and was a member of the Khartum Temple Patrol. He was an active member of the Fort Rogue Curling Club for many years. Jack was active in the community and gave fifty-six donations of blood to the Red Cross Society. Jack will be sadly missed by his daughter, Sandra Nelken; son Donald Kotak; and grandchildren, Candace Wind, Carmen Sherby, Rachel Nelken, and Andrea Cusitar. He was predeceased by his son, Leonard Kotak. Memorial service will be held on Monday, August 10 at 11:00 a.m. in the chapel of Voyage Funeral Home, 1531 Pembina Hwy.
